Enrollment Process
We appreciate your interest in join our center!
Step 1:
Please call our center at (562) 269 - 4355 to verify your information, eligibility and location.
Step 2:
Schedule an in-person tour at our center to know our services. We provide free pick up and drop off transportation for you to visit our center.
Step 3:
Please remember to bring your photo ID, Medi-cal card, Medicare card (if application), SSN, health insurance card, and your primary care physician's information with you.
Step 4:
After you return home, ask your primary health physician to fill out the history and physical examination documents and return the form to us.
Step 5:
Once we receive your history and physical examination form, our center's staff will follow up with your to start the application process.
